A Clinical Pharmacist plays a critical role in optimizing patient care through medication management and consultation. Working closely with physicians, nurses, and other healthcare providers, the Clinical Pharmacist is responsible for ensuring that patients receive the most effective medication therapies to manage their health conditions. They work in a variety of settings, including hospitals, clinics, and healthcare facilities, providing direct patient care, making clinical decisions, and educating healthcare staff.

Responsibilities:

• Collaborate with physicians and other healthcare professionals to develop and implement medication therapy plans for patients
• Review patient medical histories and medication regimens to ensure effectiveness, reduce adverse reactions, and avoid drug interactions
• Provide clinical consultation to healthcare providers regarding drug therapies, dosages, potential side effects, and interactions
• Counsel patients on proper medication use, adherence, and possible side effects
• Monitor patient progress and adjust medication plans as necessary based on laboratory results and clinical findings
• Conduct medication reconciliation to ensure accuracy of patient medication lists across healthcare transitions (e.g., hospital admission and discharge)
• Serve as a resource for healthcare staff on pharmacotherapy and best practices in medication management
• Participate in multidisciplinary healthcare team meetings to discuss patient care and optimize therapeutic outcomes
• Educate patients and caregivers on chronic disease management, lifestyle modifications, and drug therapy plans
• Oversee and guide pharmacy technicians in the preparation and dispensing of medications
• Conduct clinical research or participate in clinical trials to advance pharmaceutical knowledge and improve treatment protocols
• Develop and implement policies and procedures to ensure the safe, effective, and economical use of medications
• Stay up-to-date with advancements in pharmaceutical science, new medications, and updated clinical guidelines
• Ensure compliance with all relevant regulations, including state and federal pharmacy laws, Joint Commission standards, and hospital policies
• Provide education and training to other healthcare professionals on pharmacotherapy, safe prescribing practices, and medication safety

Requirements:

• Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school
• Valid state pharmacy license and certification as required by state law
• Completed a pharmacy residency program (PGY1 or PGY2) in a clinical setting is preferred
• Board certification in pharmacotherapy (BCPS) or other relevant certifications (e.g., BCACP, BCCCP, BCIDP) is a plus
• Strong knowledge of pharmacology, drug interactions, and therapeutic drug monitoring
• Excellent communication skills, with the ability to explain complex medical information to patients and healthcare professionals
• Proficiency in using electronic health records (EHR) and other healthcare technologies for patient data management and drug information retrieval
• Attention to detail and the ability to work in a fast-paced, patient-centered environment
• Strong analytical and problem-solving skills, with the ability to make informed clinical decisions
• Ability to work effectively as part of a multidisciplinary healthcare team
• Commitment to continuing education and staying current on advancements in clinical pharmacy

Preferred Skills:

• Experience in a hospital or clinical pharmacy setting
• Knowledge of disease state management for conditions such as diabetes, hypertension, and heart failure
• Experience in managing complex medication therapies, including biologics and specialty medications
• Bilingual skills, depending on the patient population
